How to create an NPI

Because I'm often asked this, thought I would post this as a blog for quick reference....



Go to this website:

Click on create a new account. 

Click accept

Click create account now on the right hand side of the screen

Follow the steps entering your personal email address . 

Next page will ask you to create a username and select a password. Also there are 5 security questions to answer. 

Final page request your information. Enter your home address here as well. Business number will be the company's phone number.


Once the login is made, then go back here and sign in with that info:

The first photo to the left says: apply for an NPI for myself - click there. 

One of the tricky selections in the application is selecting the taxonomy number, make sure you select your type of provider category only, without any details. You can use your home address for your personal info, but for the business address please fill out the clinic's address.

Once that is filled out, they will email you immediately, and usually within the hour email you again with the actual NPI number.
