Today is the day! Most insurance companies as of today are requiring the new CMS-1500 02/12 forms instead of the old version (CMS-1500 08/05). With this new verison, options are given to allow DSM-5/ICD-10 diagnosis coding as well as ICD-9 if necessary at this time. Debate has been circulating about whether the deadline will be delayed another year from October 1st, 2014 for the transistion of ICD-10 only, but no changes have been officially annouced as of yet.
The NUCC site has a power point presenation
(http://www.nucc.org/images/stories/PDF/understanding_the_changes_to_the_0212_1500_claim_form.pdf ) that can be viewed to show all details of the changes to the new forms.
I have found great prices at various places online to order the new claim forms (Amazon to Medical Arts Press to bookstore.gpo.gov). Prices vary and depending on your bulk order can effect that pricing. Paper claims are often only used in Workers Comp claims, sending to smaller insurance companies who are not EDI enrolled, and for corrections needing to be made to electronic claims already sent.
